Other Healthcare Activities in Germany industry analysis

On average, the industry recorded an annual increase in turnover of 17.7% between 2020 and 2025. In January 2023, the list of diagnoses was expanded, meaning that long-term prescriptions can now be issued for more illnesses. This had a positive impact on service utilisation and sales. Prices for physiotherapy and physiotherapy treatments also increased. However, these increases in turnover were almost completely eaten up by inflation: The increases in remuneration were not enough to offset the inflationary price pressure. In light of the cost increases in 2023 and 2024, the physiotherapy associations and the National Association of Statutory Health Insurance Funds have agreed on an increase in physiotherapy service remuneration of 6.4% (8.5%), starting in January 2024 (2023). The new state aid regulations are likely to provide an impetus for growth. Since November 2024, remedies in the field of physiotherapy have been reimbursable as part of a blank prescription, while occupational therapy has been reimbursable since August. This means that practices can invoice their services directly to the health insurance companies and do not require a new medical prescription. This new room for manoeuvre should have a positive impact on sales development. In 2025, the sector is expected to generate turnover of €40.9 billion, which corresponds to growth of 0.9% compared to the previous year. From 2025, German companies must be able to receive electronic invoices. However, the mandatory use does not apply to tax-free services, such as medical services, small amounts and invoices to private patients. The industry will benefit from the rising demand for healthcare services in the coming years. Due to the ageing population and the increased willingness of patients to make more co-payments for healthcare services, annual revenue growth of 2.1% on average is expected in the period from 2025 to 2030 and industry revenue of 45.3 billion euros in 2030. One challenge is the shortage of skilled labour, which is jeopardising medical care for the growing number of patients. The high bureaucratic administrative burden ties up staff and further exacerbates this problem. Digitalisation and telematics infrastructure are having an increasing impact on the development of the healthcare professions and will contribute to improving healthcare. At the same time, the implementation of digital processes can relieve the burden on staff and better counter the shortage of skilled labour. The challenges will lie in information and data security.

Trends and Insights

  • Until now, patients have not been able to receive remedial therapy without a doctor's prescription. An alternative would be direct access. By giving patients direct access to remedial therapy, the quality and cost-effectiveness of patient care could be improved.
  • With a growing proportion of people aged 65 and over, the consumer market is likely to soon account for the largest share of sales. As people get older, the risk of diseases such as strokes and chronic illnesses increases.
  • As the most populous region, the centre region has the highest proportion of companies in the sector. North Rhine-Westphalia stands out here, with the highest number of companies per federal state in Germany as a whole.
  • Physiotherapists are not allowed to advertise a particular treatment or product. Factors such as the practice location or free parking spaces and a good reputation therefore play an important role in competition.
